The Case Management staff at Great Plains Regional Medical Center assists patients and their families in dealing with the emotional, financial and social stresses that accompany illness and hospitalization.
Case managers offer help with assessing services and advance directives. They provide consultation in the areas of child and elderly abuse, domestic violence and adoption. They assist the patient in discharge planning for home care or transfer to a nursing home. They offer emotional support and counseling and make referrals to community agencies.
All case managers have a working knowledge of social work theories, therapies and techniques at a level acquired through the completion of bachelor's or master's degrees in social work or related human services field.
