The Health Information Management Department (Medical Records) works diligently to protect health information and patient confidentiality and is the keeper of the patient's medical record.

Copies of your records may be made available to you but requests cannot be received or sent via email due to HIPAA privacy and security concerns.  A sample request form is listed under the tab "Patient Resources" along with GPRMC's Privacy Policy.

Your privacy is protected and you will be given the opportunity at registration to be listed in the facility directory, send notices of your admission/discharge to the local newspaper, or participate in the Patient Identification Number (PIN) Program which allows those you choose to receive detailed medical information regarding your stay.  You may also choose not to participate in any of the programs and keep your information private.

GPRMC also participates in NEHII, in which information is sent to a protected statewide patient information database that is available to your physician or other health care provider.  Those patients who do not wish to participate should contact the Registration Department when scheduled or receiving treatment.
