Home Care Services -- Healthy Start


Great Plains Home Care Services include Home Health, Hospice care and Healthy Start. Home Health Care includes services of Visiting Nurses, Therapies, Home Health Aides and Medical Social Services. Home Health offers a convenient and comfortable way to recover from and illness, injury or surgery. Hospice is a philosophy of care that provides support to terminally ill persons and their families. Our Hospice Team empowers individuals with terminal illnesses to experience life to the fullest with dignity and comfort. Healthy Start offers educational home visits for pregnant women and families with children from birth to five years of age who meet criteria.

When you’re expecting
Before your baby’s birth, we can help you have a healthy pregnancy.

Our prenatal goals are:
• Provide education regarding fetal growth and development
• Provide education in nutrition and healthy lifestyles during pregnancy
• Provide education in preparing for infant’s arrival
• Promote compliance with scheduled OB visits

Prevention
We help you be alert to your child’s development needs. We show you how to make your home safer for your child. We educate you in getting your child’s shots and well-baby checkups.

Support
We will provide information to help you through a family crisis. We offer one-on-one teaching on parent skills. We send you to the right community agencies when you need services.

Information
We can show you positive ways to be a good parent. We teach you ways to use play to help your child develop. We offer tips on cleanliness and what foods are best for your child.

How it works
A nurse visits with you in the comfort of your own home and assesses your child’s development. She listens to your concerns, answers your questions, and shares the latest ideas about child development, health and injury prevention. Your home visits are scheduled with your convenience in mind.

Who can use this service
Healthy Start is available to pregnant women or families with newborns and children up to age five who live within Lincoln County. As a new parent, every new step with your baby is an opportunity to grow and learn. Healthy Start can assist you with those steps.
